Ovarian cysts are fluid-filled sacs that develop on or inside the ovaries and are among the most common gynecological problems in women of reproductive age. This study aimed to determine the factors associated with the incidence of ovarian cysts at the Midwifery Polyclinic of Bhayangkara Hospital. This study used an analytic observational method with a cross-sectional design. Data were collected using questionnaires and observations, then analyzed using the Chi-Square test with a 95% confidence interval (CI) and a significance level of p < 0.05. The results showed that body mass index (BMI) (p = 0.006), physical activity (p = 0.045), age at menarche (p = 0.044), menstrual cycle (p = 0.003), and stress (p = 0.025) were associated with ovarian cyst incidence. Logistic regression analysis indicated that BMI, physical activity, and stress significantly influenced ovarian cyst incidence (p < 0.05), while age at menarche and menstrual cycle were not significant. BMI was the most dominant factor with an OR value of 3.889. The Nagelkerke R Square value was 0.468, indicating that the independent variables explained 46.8% of the variation in ovarian cyst incidence. In conclusion, BMI, physical activity, age at menarche, menstrual cycle, and stress are associated with ovarian cyst incidence
