Abstract

Latar Belakang : Komplikasi persalinan masih berkontribusi besar terhadap kematian ibu di Indonesia, yang diperburuk oleh lemahnya sistem rujukan dan ketimpangan pelayanan antarwilayah. Tujuan: Penelitian ini bertujuan memetakan distribusi komplikasi persalinan dan kegagalan rujukan di 38 provinsi serta mengidentifikasi provinsi prioritas dengan beban komplikasi tinggi dan rujukan rendah melalui analisis kuadran. Metode: Studi ekologi ini menggunakan data sekunder SKI 2023 dengan unit analisis provinsi (n=38) dan sampel 7.842 perempuan yang melahirkan pada 2023. Analisis dilakukan secara agregat. Hasil: Rerata ibu mengalami sedikitnya satu komplikasi persalinan sebesar 15,5% dengan variasi antarprovinsi yang tinggi, sementara proporsi tidak dirujuk mencapai 30,5%. Korelasi antara komplikasi dan tidak dirujuk lemah dan tidak signifikan (? = -0,267; p=0,105), dan enam provinsi masuk kuadran risiko tinggi. Simpulan: Komplikasi persalinan dan kegagalan rujukan tidak selalu berbanding lurus antarprovinsi, sehingga intervensi perlu disesuaikan dengan profil kuadran masing-masing wilayah. Saran: Perlu pengembangan kebijakan berbasis kuadran untuk memprioritaskan provinsi dengan beban komplikasi tinggi dan rujukan rendah.
