Perilaku Seksual dan Penggunaan Pre-Exposure Prophylaxis (PrEP) pada Lelaki Seks Lelaki: Systematic Literature Review

Latar Belakang: Lelaki seks dengan lelaki (LSL) merupakan populasi kunci dengan risiko tinggi HIV di Asia Tenggara. Tercatat sekitar 300.000 infeksi HIV baru di kawasan ini pada tahun 2024, namun pemanfaatan Pre-Exposure Prophylaxis (PrEP) sebagai intervensi pencegahan efektif masih belum optimal akibat hambatan struktural dan sosial. Tujuan: Menganalisis hubungan perilaku seksual berisiko dengan penggunaan PrEP, serta mengidentifikasi faktor sosial, struktural, dan layanan kesehatan yang memengaruhi uptake dan keberlanjutan PrEP pada LSL di Asia Tenggara. Metode: Systematic Literature Review (SLR) berbasis pedoman PRISMA dan kerangka PICOS dengan penelusuran pada PubMed dan ScienceDirect, periode 2015–2025, 10 studi memenuhi kriteria inklusi dan dianalisis melalui sintesis tematik. Hasil: Perilaku seksual berisiko (chemsex, hubungan anal tanpa kondom, multiple sexual partners), persepsi risiko HIV tinggi, dan pengetahuan PrEP meningkatkan uptake PrEP. Kepatuhan cenderung menurun dalam jangka panjang. Stigma sosial, biaya, dan ketimpangan akses layanan menjadi hambatan utama. Simpulan: Perilaku seksual berisiko berkorelasi positif dengan penggunaan PrEP pada LSL; namun hambatan struktural dan sosial secara signifikan membatasi aksesibilitas dan keberlanjutannya di Asia Tenggara. Saran: Diperlukan penguatan layanan berbasis komunitas, integrasi PrEP dalam program HIV nasional.
